Explanation from ACUA first and then an application form is reproduced. Website users should be able to print out the form.
First, right-click on the image of the form to save it as a picture offline.
Second, import it to MS-Word as an image.
Third, re-size it to a standard 8 X 11 piece of paper.
Finally, print.

The forms are available at City Hall. Also, the City Clerk would be able to send the forms as an attachment to e-mail. Those interested in obtaining this service can also contact WeLoveVentnor@gmail.com and forms will be sent by e-mail.
